Sliding Fee Discount Program Policy

Patrick County Family Practice patients who are uninsured or underinsured can apply for services at a reduced rate.*

* These rates and policies DO NOT apply to Patrick County Urgent Care

This rate is based on the total income of the household, the number of people in the household, compared to the yearly Federal poverty guideline.

Patrick County Family Practice can not refuse treatment for anyone who can not pay.

It is the policy of Patrick County Family Practice to provide essential services regardless of the patient’s ability to pay. PCFP offers discounts based on family size and annual income.

Please complete the Sliding Scale Application and bring to the front desk to determine if you or members of your family are eligible for a discount.

The discount will apply to all services received at this clinic, but not those services or equipment purchased from outside, including reference laboratory testing, drugs, and x-ray interpretation by a consulting radiologist, and other such services. You must complete this form every 12 months or if your financial situation changes.
